  • Clinical significance of the changes of circFOXP1 and circMAP3K4 gene expression in serum of pregnant women with diabetes

    Objective To explore the changes and clinical significance of serum cyclic RNA forkhead box protein P1(circFOXP1) and circMAP3K4 gene expression in patients with gestational diabetes(GDM). Methods From November 2021 to November 2022, 128 GDM patients in the Department of Obstetrics and Gynecology of Hanyang Hospital Affiliated to Wuhan University of Science and Technology were selected as the GDM group, and 128 healthy pregnant women in the hospital were selected as the control group at the same time. Real time fluorescent quantitative PCR(qPCR) was used to detect the expression of circFOXP1 and circMAP3K4 genes in serum; Pearson method was used to analyze the correlation between the expression of circFOXP1 and circMAP3K4 in serum of GDM patients. The diagnostic value of serum circFOXP1 and circMAP3K4 in GDM was analyzed using receiver operating characteristic curve(ROC) analysis. Logistic regression analysis of factors affecting the occurrence of GDM. Results Compared with the control group, the levels of triglyceride(TG), low-density lipoprotein cholesterol(LDL-C), fasting plasma glucose(FPG), and fasting insulin(FINS) in the GDM group were significantly increased, and the levels of High-density lipoprotein cholesterol(HDL-C) were significantly reduced(t/P=23.019/<0.001, 7.081/<0.001, 36.805/<0.001, 22.069/<0.001, 3.341/0.001). Compared with the control group, the GDM group showed a decrease in serum circFOXP1 gene expression and an increase in circMAP3K4 gene expression(t/P=11.630/<0.001, 12.061/<0.001). There is a negative correlation between the expression of circFOXP1 and circMAP3K4 gene in the serum of GDM patients(r/P=-0.302/0.001). The combined diagnosis of circFOXP1 and circMAP3K4 in serum resulted in a higher incidence of AUC in GDM compared to individual diagnosis(Z/P=2.207/<0.001, 2.029/0.023). High FPG expression, low circFOXP1 expression, and high circMAP3K4 expression are risk factors for GDM occurrence [OR(95% CI)=2.017(1.280-3.178), 2.955(1.454-6.008), 3.154(1.735-5.734)].Conclusion The serum of GDM patients shows low expression of circFOXP1 and high expression of circMAP3K4, which can serve as auxiliary diagnostic markers for GDM.
  • Serum PCSK9 and CRTC3 levels in pregnant women with polycystic ovary syndrome and their predictive value for adverse pregnancy

    Objective To investigate the levels of serum pre protein converting enzyme Bacillus subtilis lysozyme 9(PCSK9) and transcriptional co activator 3(CRTC3) regulated by cyclic adenosine responsive element binding protein in pregnant women with polycystic ovary syndrome(PCOS) and their predictive value for adverse pregnancy. Methods One hundred and twenty-three pregnant women with PCOS admitted to the Obstetrics and Gynecology Department of Wuhan Third Hospital from December 2020 to September 2022 were included as the study group, while 120 healthy pregnant women who underwent pregnancy examination and gave birth at the hospital were selected as the control group during the same period. PCOS pregnant women were divided into an adverse pregnancy subgroup(32 cases) and a good pregnancy subgroup(91 cases) based on whether adverse pregnancy outcomes occurred. Enzyme linked immunosorbent assay(ELISA) was used to measure the serum levels of PCSK9 and CRTC3 in two groups of study subjects. Pearson method was used to analyze the correlation between serum PCSK9 and CRTC3 levels in pregnant women with PCOS. Multivariate logistic regression analysis of the influencing factors of adverse pregnancy outcomes in PCOS patients. The predictive value of serum PCSK9 and CRTC3 levels on adverse pregnancy outcomes in PCOS pregnant women was analyzed using the receiver operating characteristic(ROC) curve, and the area under the curve(AUC) was calculated and compared.Results Compared with the control group, the levels of PCSK9, CRTC3, testosterone(T), luteinizing hormone(LH), fasting blood glucose(FPG), fasting insulin(FINS), and insulin resistance index(HOMA-IR) in the study group of PCOS patients were significantly increased(t/P=8.611/<0.001, 7.794/<0.001, 29.474/<0.001, 73.271/<0.001, 6.258/<0.001, 24.295/<0.001, 16.511/<0.001); Compared with the good pregnancy subgroup, the levels of PCSK9 and CRTC3 in the poor pregnancy subgroup of PCOS patients were significantly increased(t/P=7.449/<0.001, 7.943/<0.001). Pearson method analysis showed a positive correlation between serum PCSK9 and CRTC3 levels in PCOS patients(r=0.639, P<0.001). The results of multivariate logistic regression analysis showed that PCSK9 and CRTC3 were risk factors for adverse pregnancy outcomes in PCOS pregnant women [OR(95%CI)=4.152(2.288-7.534), 10.369(2.422-44.396)]. The AUC of serum PCSK9, CRTC3, and their combination in predicting adverse pregnancy outcomes is better than that of PCSK9, CRTC3 alone(Z/P=2.618/0.009, 3.104/0.001). Conclusion The serum levels of PCSK9 and CRTC3 in pregnant women with PCOS significantly increased, which are closely related to pregnancy outcomes. The combined detection of the two has important value in predicting adverse pregnancy outcomes.
  • Exploring the prognostic value of serological indicators for cervical cancer based on a survival prediction model using column charts

    Objective To explore the high-risk factors for prognosis of cervical cancer patients, and establish a column chart based on serological tumor markers to predict the prognosis of cervical cancer patients. Methods Two hundred and sixty-eight patients with cervical cancer who underwent gynecological surgery at the People's Hospital of Xinjiang Uygur Autonomous Region from March 2019 to March 2021 were selected as the research subjects. Their clinical data were analyzed and divided into a survival group of 226 cases and a death group of 42 cases based on 2-year follow-up. Serum indicators such as carbohydrate antigen-125(CA125), human epididymal protein 4(HE4), and squamous cell carcinoma antigen(SCC-Ag) were detected using chemiluminescence assay. Serum indicators such as alpha fetoprotein(AFP), carcinoembryonic antigen(CEA), and tumor specific growth factor(TSGF) levels were detected using enzyme-linked immunosorbent assay; Using multivariate Cox regression analysis to determine the prognostic factors of cervical cancer patients; Based on the results of multiple factor analysis, a column chart was constructed, and the consistency and discrimination of the column chart model were evaluated using calibration curves and receiver operating characteristic(ROC) curves. Results The levels of SCC-Ag and TSGF in the death group, as well as the proportion of poorly differentiated and stage III C1 patients, were higher than those in the survival group [t(χ2)/P=9.636/<0.001, 8.237/<0.001, 13.590/<0.001, 13.604/<0.001]. Low tissue differentiation, stage IIIC1, high SCC-Ag, and TSGF are independent risk factors for mortality in cervical cancer patients [HR(95%CI)=4.694(1.628-13.538), 6.926(2.367-20.266), 9.658(4.216-22.129), 1.071(1.040-1.104),P<0.01].The calibration curve shows that the actual values of the column chart model are basically consistent with the predicted values, and the area under the curve of the ROC curve is 0.912(95%CI 0.863-0.962). Conclusion A column chart model constructed by combining serum indicators SCC-Ag and TSGF with tissue differentiation and staging is beneficial for early assessment of the risk of 2-year mortality in cervical cancer patients.
  • The value of transvaginal ultrasound combined with elastography in the differentiation of uterine cavity lesions

    Objective To explore the value of transvaginal ultrasound combined with elastography in the differentiation of uterine cavity lesions. Methods One hundred and twenty-six patients with uterine cavity lesions who were treated in the Gynecology Department of the Second Affiliated Hospital of Hebei North University from January 2018 to January 2021 were selected for transvaginal ultrasound and transvaginal elastography using the GE-E10 color Doppler ultrasound diagnostic instrument. The probe frequency was 5-9 MHz, and the histopathological examination results were used as the "gold standard" to compare the results of transvaginal ultrasound, elastography, and combined examination, respectively, Simultaneously record the average elastic value(Emea), maximum elastic value(Emax), minimum elastic value(Emin), and standard deviation of elasticity(Esd) of the diseased uterine cavity tissue, compare the differences in elastic modulus values between benign and malignant uterine cavity lesions, draw the receiver operating characteristic curve(ROC) of each elastic modulus value for diagnosing benign and malignant uterine cavity lesions, and analyze the application value of vaginal ultrasound combined with elastic imaging in the differential diagnosis of benign and malignant uterine cavity lesions. Results The Emea, Emax, Emin, and Esd of elastic imaging in the malignant lesion group were higher than those in the benign lesion group, with statistical significance(t=28.573, 23.996, 2.618, 17.412, P<0.01); The coincidence rate between malignant lesions detected by transvaginal ultrasound and histopathological diagnosis was 54.3%, elastic imaging examination was 71.4%, and combined examination was 88.6%; The coincidence rate between benign lesions detected by transvaginal ultrasound and histopathological diagnosis was 82.1%, 78.6% by elastography, and 92.9% by combined examination. The AUC of benign and malignant lesions diagnosed by transvaginal ultrasound was 0.538, the AUC of elastography was 0.661, and the AUC of combined examination was 0.788. The AUC of combined examination was higher than that of transvaginal ultrasound and elastography(P=0.04). Conclusions Transvaginal ultrasound combined with ultrasound elastography technology can improve the diagnostic accuracy of uterine cavity malignant lesions.
  • Based on PI3K/AKT/NF- κ exploring the protective effect of self-made Yinghuang Decoction on sepsis rats through the B signaling pathway

    Objective To investigate the protective effect of Yinghuang Decoction on septic rats based on PI3K/AKT/NF-κB signaling pathway. Methods Two hundred and forty male SD rats were randomly divided into sham-operated group, model group, dexamethasone group, Yinghuang Decoction low-dose group, Yinghuang Decoction medium-dose group and Yinghuang Decoction high-dose group, 40 rats in each group. Twenty rats in each group were used to observe the survival rate within 7 days. After the remaining 20 rats were molded and administered for 72h, the therapeutic effects of YingHuang Decoction were observed by detecting the coagulation-related indexes, wet weight/dry weight ratio of lung tissues, total protein content in BALF, and liver and kidney function-related indexes; the pathological changes of YingHuang Decoction on lung, liver and kidney tissues were observed by HE staining; the levels of IL-6, IL-1β and TNF-α in serum and the levels of IL-6, The effect of YingHuang Decoction on inflammatory factors was observed by detecting serum levels of IL-6, IL-1β, TNF-α and gene expression of IL-6,IL-1β and TNF-α in lung tissues; the effect of Yinghuang Decoction on PI3K/AKT/NF-κB pathway in lung tissues was detected by Western-blot method. Results The survival rate of septic rats in Decoction medium-dose group and high-dose group was higher than that in dexamethasone group. After modeling administration for 72h, compared with the sham-operated group, the serum levels of PLT and Fib in the model group rats were significantly reduced, and the PT, APTT and TT times were significantly prolonged; the levels of total protein, W/D ratio, ALT, AST, SCr and BUN in BALF were significantly increased; the histopathology of lung, liver and kidney were significantly damaged; the serum levels of IL-6, IL-1β, TNF-α,IL-6,IL-1β,TNF-α gene levels were significantly increased in lung tissue; p-PI3K/PI3K, p-AKT/AKT, p-P65/P65 protein ratios were increased. Compared with the model group, the serum levels of PLT and Fib were significantly increased(F/P=39.161/0.000, 11.233/0.000) and PT and APTT times were significantly shortened(F/P=32.933/0.000, 22.177/0.000) in the dexamethasone and YingHuang Tang groups at each dose; total protein in BALF, W/D ratios, the ALT, AST, SCr, and BUN levels were significantly reduced(F/P=27.160/0.000, 29.291/0.000, 20.553/0.000, 10.567/0.000, 28.450/0.000, 11.374/0.000); lung, liver, and kidney tissue damage was significantly improved; serum IL-6, IL-1β, TNF-α levels were significantly reduced(F/P=26.967/0.000, 38.238/0.000, 34.3333/0.000), and IL-6,IL-1β, TNF-α gene expression was reduced in lung tissue; p-PI3K/PI3K, p-AKT/AKT, p-P65/P65 ratios were significantly reduced(F/P=22.826/0.000, 21.432/0.000, 83.294/0.000). Conclusion Yinghuang Decoction can improve coagulation dysfunction, protect lung, liver and kidney function and reduce inflammatory levels in CLP rats, and these changes may be related to the inhibition of PI3K/AKT/NF-κB signaling pathway activation.
