  • Efficacy of single-port thoracoscopic segmentectomy for non-small cell lung cancer patients and its effects on pain mediator levels and short-term outcomes

    Objective To analyze and evaluate the efficacy of single-port thoracoscopic segmentectomy for nonsmall cell lung cancer patients, and its impact on pain mediator levels and short-term prognosis. Methods From January2022 to June 2025, the Department of Thoracic Surgery at the First Affiliated Hospital of Hebei North University enrolled 98 patients with non-small cell lung cancer(NSCLC) as study subjects. These patients were divided into a study group(54 patients) and a conventional group(44 patients) based on their treatment modalities. The conventional group underwent conventional three-port thoracoscopic segmentectomy, while the study group underwent single-port thoracoscopic segmentectomy. The two groups were compared in terms of surgical outcomes, pain levels, pulmonary function, VAS and 6 MWT scores, MMSE scores, complications, and short-term prognosis. Results In the postoperative study group, intraoperative blood loss, drainage volume, and levels of Cor, PGE2, NE, NPY, SP, SCC-Ag, CYFRA21-1, CEA, and CA50, as well as VAS scores, were all significantly lower than those in the conventional group(t = 6.648, 9.457, 7.073, 5.363, 2.792, 3.199, 3.160, 6.253, 6.339, 3.057,2.295, 4.850; all P <0.05).The time to extubation,operative time, the levels of DLco, 5-HT, MVV, FVC, and FEV1, as well as the 6 MWT and MMSE scores, were all significantly higher in the study group than in the conventional group(t = 5.463, 4.724,4.420, 4.118, 5.333, 2.602, 6.760; all P <0.05). The overall incidence of complications in the conventional group was 25.00%(11/44), which was significantly higher than that in the study group(7.41%, 4/54)(χ2= 5.789,P = 0.016). The recurrence rate and distant metastasis rate in the conventional group were 6.82%(3/44) and 9.09%(4/44), respectively, while those in the study group were 1.85%(1/54) and 3.70%(2/54). The differences between the two groups were not statistically significant(χ2= 0.522,P = 0.470; χ2= 0.466,P = 0.495). No deaths were observed in either group. Conclusion Single-port thoracoscopic segmentectomy demonstrates favorable therapeutic outcomes in patients with non-small cell lung cancer. It can reduce damage to lung function and surgical trauma, lower levels of pain mediators, and improve early cognitive impairment, facilitating faster postoperative recovery for patients. Moreover, the risk of complications did not increase significantly, making it safe and feasible.
  • The expression of KDM1A,POSTN in thyroid carcinoma and their correlation with tumor stem genes and clinical significance

    Objective To study the expression of lysine demethylase 1A(KDM1A) and periostin(POSTN) in papillary thyroid carcinoma(PTC) and their correlation with tumor stemness genes and clinical significance. Methods From January 2018 to February 2019, 98 patients with PTC were diagnosed and treated in the Department of Thyroid and Breast Surgery, Chaohu Hospital Affiliated to Anhui Medical University. Quantitative real-time PCR was used to detect the expression of KDM1A m RNA, POSTN m RNA, and tumor stemness genes [octamer-binding transcription factor 4(OCT4)m RNA, sex-determining region Y-box protein 2(SOX2) m RNA, and NANOG m RNA]. The expression of KDM1A and POSTN proteins was detected by immunohistochemistry. Pearson correlation analysis was used to analyze the correlation between KDM1A m RNA, POSTN m RNA and OCT4 m RNA, SOX2 m RNA, NANOG m RNA. Kaplan-Meier survival analysis and multivariate Cox regression were used to evaluate the prognostic significance of KDM1A and POSTN protein expression in PTC patients. Results The expression levels of KDM1A m RNA, POSTN m RNA, OCT4 m RNA, SOX2 m RNA, and NANOG m RNA in PTC cancer tissues were significantly higher than those in adjacent normal tissues(t/P = 42.906/<0.001,33.516/<0.001, 36.439/<0.001, 36.146/<0.001, 35.229/<0.001). KDM1A m RNA and POSTN m RNA were positively correlated with OCT4 m RNA, SOX2 m RNA, and NANOG m RNA expression in PTC(r = 0.722/<0.001, 0.683/<0.001, 0.701/<0.001;0.654/<0.001, 0.721/<0.001, 0.767/<0.001). The positive rates of KDM1A and POSTN in TNM stage Ⅲ–Ⅳ and lymph node metastasis were significantly higher than those in stage Ⅰ– Ⅱ and without lymph node metastasis(χ2/P = 15.938/<0.001,8.284/0.004, 10.542/0.001, 9.503/0.002). The 5-year progression-free survival(PFS) rate of KDM1A-positive patients was significantly lower than that of KDM1A-negative patients [64.52%(40/62) vs. 88.89%(32/36)](Log-rank χ2= 4.160, P =0.013). The 5-year PFS rate of POSTN-positive patients was significantly lower than that of POSTN-negative patients[65.63%(42/64) vs. 88.24%(30/34)](Log-rank χ2= 3.864, P = 0.041). TNM stage Ⅲ–Ⅳ, lymph node metastasis, high KDM1A, and high POSTN were independent risk factors for poor prognosis of PTC [OR(95%CI) = 1.706(1.208-2.408),1.511(1.122-2.036), 1.442(1.113-1.868), 1.449(1.152-1.823)]. Conclusion The expression of KDM1A and POSTN m RNA and protein is significantly increased in PTC, which is positively correlated with the expression of tumor stemness genes,and they are novel indicators for evaluating the prognosis of PTC.
  • Study on the correlation of LAG3,FGL1 expression levels and immune checkpoint molecules in laryngeal cancer tissue and prognostic value

    Objective To study the relationship between the expression of lymphocyte activation gene 3(LAG3), fibrinogen-like protein 1(FGL1) and immune checkpoint molecules and prognosis in laryngeal cancer. Methods The expression of LAG3 m RNA, FGL1 m RNA, and immune checkpoint molecules [programmed death factor 1(PD-1) m RNA, programmed death factor ligand 1(PD-L1) m RNA]in laryngeal cancer tissues was detected by q PCR. Pearson correlation analysis was used to analyze the correlation between LAG3 m RNA, FGL1 m RNA expression and immune checkpoint molecules in laryngeal cancer tissues. The expression of LAG3 and FGL1 protein in laryngeal carcinoma tissues was detected by immunohistochemistry. Kaplan-Meier curve and Cox regression model were used to analyze the prognostic factors of patients with laryngeal cancer. Results The expression levels of LAG3 m RNA, FGL1 m RNA, PD-1 m RNA, and PD-L1 m RNA in laryngeal cancer tissues were significantly higher than those in adjacent normal tissues(t/P = 34.516/<0.001, 31.372/<0.001,30.335/<0.001, 23.894/<0.001). LAG3 m RNA and FGL1 m RNA in laryngeal cancer tissues were positively correlated with PD-1 m RNA and PD-L1 m RNA(r = 0.714/<0.001, 0.681/<0.001, 0.721/<0.001, 0.663/<0.001). The positive rates of LAG3 and FGL1 in laryngeal cancer tissues were 77.78%(70/90) and 75.56%(68/90), respectively, which were significantly higher than those in adjacent normal tissues [8.89%(8/90) and 6.67%(6/90)](χ2/P = 86.968/<0.001, 88.210/<0.001). The positive rates of LAG3 and FGL1 in laryngeal cancer tissues were higher in patients with TNM stage Ⅲ-Ⅳ and lymph node metastasis(χ2/P = 13.125/<0.001, 3.920/0.048, 12.995/<0.001, 11.810/0.001). The 3-year overall survival rate of the LAG3-positive group was 78.57%(55/70), which was significantly lower than that of the negative group(90.00%, 18/20). The 3-year overall survival rate of the FGL1-positive group was 77.94%(53/68), which was significantly lower than that of the negative group(90.91%, 20/22)(Log-rank χ2/P = 4.023/0.002, 3.799/0.028). TNM stage Ⅲ-Ⅳ, lymph node metastasis, LAG3 positivity, and FGL1 positivity were independent risk factors affecting the prognosis of laryngeal cancer [OR(95%CI) = 1.477(1.075-2.029),1.441(1.095-1.895), 1.259(1.033-1.534), 1.310(1.058-1.622)]. Conclusion The expression of LAG3 and FGL1 is upregulated in laryngeal cancer tissues. They are positively correlated with the expression of immune checkpoint molecules and are new markers for the prognosis of laryngeal cancer.
  • The effects of asiaticoside on the proliferation,migration and invasion of glioma cells by regulating the Ras/Raf/Mek/Erk signaling pathway

    Objective To investigate the effects of asiaticoside(Asi) regulating the Ras/Raf/Mek/Erk pathway on the proliferation, migration and invasion of glioma cells. Methods Experiments were conducted at the Hebei Provincial Key Laboratory of Cardiology and Metabolism from March 2025 to August 2025. U87 cells were divided into the U87 cell group,low-dose and high-dose asiaticoside(Asi-L, Asi-H) groups, activator(ML-098) group, and high-dose asiaticoside + pathway activator(Asi-H+ML-098) group. Cell proliferation activity was detected by CCK-8 assay and Ed U assay, and cell apoptosis was detected by flow cytometry. The scratch assay was used to measure cell migration ability, and the Transwell chamber was used to measure cell invasion activity. The expression of Ras/Raf/Mek/Erk pathway proteins was detected by Western blot.U87 cells were injected into nude mice to construct xenograft tumor models, and the mice were divided into the model group,low-dose and high-dose asiaticoside(Asi-L, Asi-H) groups. Tumor volume and weight were measured, and the basic morphological structure of the tumor tissue was observed by HE staining. The positive expressions of Ki-67 and cleaved caspase-3 were detected by immunohistochemistry. In addition, the protein expressions of the Ras/Raf/Mek/Erk pathway were detected by Western blot. Results Compared with the U87 cell group, the Asi-L and Asi-H groups exhibited decreased U87 cell viability, Ed U-positive cell rate, wound healing rate, number of invasive cells, activity of the Ras/Raf/MEK/ERK pathway, and expression of c-Myc protein, while showing increased expression of p53 protein and cell apoptosis rate(q/P = 8.981/<0.001,5.730/<0.001, 8.490/<0.001, 18.632/<0.001, 7.473/<0.001, 19.106/<0.001, 13.580/<0.001; q/P = 34.571/<0.001, 17.540/<0.001,9.460/<0.001, 19.852/<0.001, 19.702/<0.001, 27.924/<0.001, 20.096/<0.001). In contrast, the ML-098 group showed elevated cell viability, Ed U-positive cell rate, wound healing rate, number of invasive cells, activity of the Ras/Raf/MEK/ERK pathway,and expression of c-Myc protein, along with reduced expression of p53 protein and a decreased cell apoptosis rate(q/P =4.616/<0.001, 3.344/0.007, 3.573/0.005, 3.582/0.005, 14.537/<0.001, 3.753/<0.001, 4.384/0.001). Compared with the Asi-H group, the Asi-H+ML-098 group exhibited increased cell viability, Ed U-positive cell rate, wound healing rate, number of invasive cells,activity of the Ras/Raf/MEK/ERK pathway, and expression of c-Myc protein, along with decreased expression of p53 protein and a reduced cell apoptosis rate(q/P = 12. 462/< 0. 001, 11. 458/< 0. 001, 12. 734/< 0. 001, 9. 267/< 0. 001, 14. 537/< 0. 001,5.435/<0.001, 12.041/<0.001). Animal experiments showed that compared with the model group, the Asi-L and Asi-H groups exhibited reduced tumor mass and volume, lower Ki-67 positive rate, and decreased expression of Ras/Raf/MEK/ERK pathway proteins, along with an increased cleaved caspase-3 positive rate(q/P = 7.452/<0.001, 4.771/<0.001, 4.576/0.002, 6.519/<0.001,5.367/<0.001, 6.286/<0.001, 5.534/<0.001, 10.996/<0.001;q/P = 18.398/<0.001, 13.103/<0.001, 16.228/<0.001, 18.447/<0.001,11.628/<0.001, 12.655/<0.001, 12.572/<0.001, 15.244/<0.001). Conclusion Asiaticoside may inhibit the proliferation, migration, invasion, and tumor growth of glioblastoma U87 cells by suppressing the activity of the Ras/Raf/MEK/ERK pathway.

    Cardiac lipoma is a rare type of heart tumor. Its clinical manifestations vary greatly, ranging from asymptomatic to arrhythmias, outflow tract obstruction, and even mimicking myocardial infarction, which may indicate malignancy.Multimodal imaging, especially cardiac magnetic resonance, which relies on the specific signal of fat tissue, serves as a core tool for diagnosis and differential diagnosis. It can accurately assess the extent of tumor invasion and guide treatment decisions. Treatment should be individualized. For patients with symptoms or high-risk imaging features, active surgery should be performed. Resection of ventricular and invasive tumors is technically challenging and associated with poor prognosis. Due to the lack of unified diagnostic and treatment guidelines, future multi-center studies are needed to clarify the management strategies for asymptomatic patients and the threshold for surgical intervention. This article aims to provide a comprehensive review on this topic.
