Objective To evaluate the efficacy of early postoperative topical antiviral therapy in preventing residual vaginal lesions and recurrent cervical lesions in patients with high-grade squamous intraepithelial lesions(HSIL) undergoing cervical conization, and to provide evidence-based support for individualized postoperative management and improved prognosis in HSIL patients. Methods A total of 166 HSIL patients scheduled for cervical conization and admitted to the Gynecology Medical Diagnosis and Treatment Center, People's Hospital of Xinjiang Uygur Autonomous Region from June 2022 to June 2024 were enrolled. They were randomly divided into an observation group(83 cases) and a control group(83 cases)using a random number table. The control group received cervical conization alone, while the observation group received early postoperative topical antiviral therapy following conization. Baseline clinical data were collected for both groups. Surgical outcomes(operative time, intraoperative blood loss, duration of postoperative vaginal discharge, and wound healing time), residual vaginal lesion rate at 3 months post-surgery, and cervical lesion recurrence rate at 12 months post-surgery were compared between the two groups. Additionally, human papillomavirus(HPV) clearance and serum levels of inflammatory factors [interleukin-6(IL-6), tumor necrosis factor-alpha(TNF-α)]were measured before and after treatment. Adverse events during treatment were recorded. The Kaplan-Meier method was used to plot survival curves and analyze differences in recurrence-free survival rates between the two groups. Results The observation group had significantly shorter duration of postoperative vaginal discharge and wound healing time compared to the control group(t = 4.507, 5.617; both P <0.001). At 3 months post-surgery, the observation group showed lower rates of residual vaginal lesions(3.6%) and residual cervical lesions(2.4%) than the control group(14.5% and 10.8%, respectively), and a higher HPV clearance rate(86.7% vs. 73.5%;χ2= 5.936, 4.771, 4.576;P = 0.015, 0.029, 0.032). Within 12 months post-surgery, the observation group had a lower cervical lesion recurrence rate(4.8%) than the control group(16.9%). Kaplan-Meier survival analysis indicated a higher recurrence-free survival rate in the observation group(95.2%) than in the control group(83.1%), with a statistically significant difference by the log-rank test(χ2= 6.316,P = 0.012). At 3 months post-surgery, serum IL-6 and TNF-α levels in both groups were lower than before treatment, and the levels in the observation group were significantly lower than those in the control group(t = 5.143, 4.419; both P <0.001). The incidence of adverse events during treatment was 9.6% in the observation group and 7.2% in the control group, with no statistically significant difference(P >0.05). Conclusion Early postoperative topical antiviral therapy in HSIL patients undergoing cervical conization can significantly shorten postoperative recovery time, increase HPV clearance rates, reduce residual vaginal lesions and cervical lesion recurrence, and has a favorable safety profile.

Objective To explore the effect of micro RNA-132-3p(miR-132-3p) adjusting the Notch1/hairy and enhancer of split homolog 1(Hes1) pathway on intestinal injury in rats with hyperlipidemia acute pancreatitis(HLAP). Methods From March to June 2025, the experiment was conducted at Bengbu Medical University in Anhui Province. The interaction between miR-132-3p and Notch1 was verified by dual-luciferase reporter gene assay. The successfully modeled HLAP rats were randomly separated into the HLAP group, the NC agomir group(intraperitoneal injection of 50 mg/kg NC agomir), the miR-132-3p agomir group(intraperitoneal injection of 50 mg/kg miR-132-3p agomir), and the JFC group(intraperitoneal injection of 50 mg/kg miR-132-3p agomir+ gavage 0.5 mg/kg JFC), 18 rats in each group. Another 18 rats were selected as the sham group. The sham group and the HLAP group were intraperitoneally injected and intragastally administered the same amount of normal saline. The levels of amylase(AMY), lipase(LPS), Total cholesterol(TC) andtriglycerides(TG) in serum of rats in each group were detected. ELISA kits were used to measure the levels of interleukin(IL)-6, tumor necrosis factor-α(TNF-α), IL-18, diamine oxidase(DAO), and D-lactic acid(D-Lac) in rat serum. HE staining was implemented to measure the histopathology of pancreatic and small intestinal tissues in HLAP rats. The apoptosis of small intestinal tissue epithelial cells in HLAP rats was detected by the TUNEL method. The mRNA expression in the small intestinal tissue of HLAP rats was detected by q RT-PCR. In addition, Western blot was implemented to measure the protein expressions of Notch1 and Hes1 in the small intestinal tissues of HLAP rats. Results Mi R-132-3p was able to target and negatively regulate Notch1. The pancreatic and small intestinal tissue structures in the sham group were intact. In the HLAP group and the NC agomir group, pancreatic tissues showed swelling, structural disorder, bleeding, and a large number of inflammatory cell infiltrations, the small intestinal tissue was clearly disordered, with a large number of intestinal villous epithelial cells shedding and inflammatory cell infiltration visible. At the same time, the lamina propria was exposed, and the capillaries were dilated and congested. In the miR-132-3p agomir group, the pathological damage of pancreatic and small intestinal tissues was clearly alleviated, and the infiltration of inflammatory cells was prominently reduced. In the JFC group, the pathological injuries of pancreatic and small intestinal tissues were further aggravated, and the infiltration of inflammatory cells increased. The HLAP group had higher apoptosis rate, AMY, LPS, TC, TG, DAO, D-Lac, IL-18, IL-6, TNF-α, Notch1 mRNA and protein, Hes1 mRNA and protein than the sham group, and a lower miR-132-3p than the sham group(t = 46.291/<0.001, 29.962/<0.001, 33.348/<0.001, 36.566/<0.001,25.414/<0.001, 26.754/<0.001, 25.230/<0.001, 29.379/<0.001, 26.667/<0.001, 23.375/<0.001, 38.298/<0.001, 21.882/<0.001,28.379/<0.001, 18.844/<0.001, 38.090/<0.001). The miR-132-3p agomir group had lower apoptosis rate, AMY, LPS, TC, TG,DAO, D-Lac, IL-18, IL-6, TNF-α, Notch1 mRNA and protein, Hes1 mRNA and protein than the HLAP group and the NC agomir group, and a higher miR-132-3p than the HLAP group and the NC agomir group(F/P = 241.158/<0.001, 209.783/<0.001,285.3093/<0.001, 241.427/< 0.001, 104.275/< 0.001, 158.707/< 0.001, 125.111/< 0.001, 204.884/< 0.001, 194.494/< 0.001,119.613/<0.001, 233.623/<0.001, 102.852/< 0.001, 119.277/< 0.001, 72.183/< 0.001, 282.118/< 0.001). The JFC group had higher apoptosis rate, AMY, LPS, TC, TG, DAO, D-Lac, IL-18, IL-6, TNF-α, Notch1 mRNA and protein, Hes1 mRNA and protein than the miR-132-3p agomir group(t/P = 22.299/<0.001, 18.662/<0.001, 23.332/<0.001, 23.465/<0.001, 13.583/<0.001,17.268/<0.001, 17.926/<0.001, 22.529/<0.001, 22.688/<0.001, 16.519/<0.001, 24.448/<0.001, 15.630/<0.001, 13.169/<0.001,12.562/<0.001). Conclusion Mi R-132-3p inhibits intestinal injury in HLAP rats by adjusting the Notch1/Hes1 pathway.

Objective This study aims to conduct a systematic evaluation on the therapeutic effect and safety of oral Chinese herbal medicine plus external application intervention in patients with subacute thyroiditis(SAT), and offer evidencebased support for clinical management. Methods We conducted exhaustive online database searches covering internationally recognized biomedical platforms such as Pub Med, Cochrane Library, EMBASE and OVID, while complementary searches were additionally performed within domestic Chinese literature systems represented by CNKI, CBM and Wanfang Database.Randomized controlled trials(RCTs) regarding TCM therapy for subacute thyroiditis were retrieved, with the search time frame from database establishment to June 2025. According to the Cochrane systematic review methodology, methodological quality assessment and data extraction of eligible studies were conducted. Rev Man 5.4 software was adopted for Meta-analysis of outcome indicators; Stata software was used to carry out sensitivity analysis; funnel plots and Egger's test were applied to evaluate publication bias. Results A total of 18 literatures were finally included after rigorous screening. Meta-analysis outcomes demonstrated that the combined oral and external TCM treatment group possessed a markedly higher overall response rate than the Western medicine group(RR = 1.13, 95%CI [1.06, 1.20],P = 0.0003); the TCM syndrome score was significantly decreased(MD =-3.63, 95%CI [-4.70,-2.56],P <0.00001), while the overall efficacy of TCM syndromes showed no statistical difference between the two groups(RR = 1.11, 95%CI [1.00, 1.23],P = 0.06).The final level of FT3 in the integrated TCM treatment group was lower than that in the Western medicine group(SMD =-0.26, 95%CI [-0.51,-0.01],P = 0.04), whereas no significant differences were found in the final FT4 and TSH levels between the two groups(P >0.05). There was no statistical difference in reducing erythrocyte sedimentation rate(ESR) and C-reactive protein(CRP) levels between the two treatment regimens(P >0.05). The incidence of hypothyroidism in the TCM combined treatment group was obviously lower compared with the Western medicine group(RR = 0.30, 95%CI [0.16, 0.60],P <0.0006).Conclusion Oral TCM combined with external application exhibits superior effects in improving overall clinical efficacy, alleviating TCM syndromes, reducing FT3 levels and lowering the incidence of hypothyroidism for SAT treatment when compared with Western medicine. Nevertheless, limited by the unsatisfactory methodological quality and substantial heterogeneity of included trials, more high-quality RCTs are required to further verify its clinical efficacy and safety.
