  • Comparison of clinical efficacy between ultrasound-guided vacuum assisted circumcision and traditional open surgery for the treatment of breast fibroma patients

    Objective To explore the curative effect of ultrasound-guided vacuum-assisted excision on mammary fibroma and its influences on postoperative recovery time and sensory nerve function in areola region.Methods A total of 120 patients with breast fibroma admitted to the Thyroid and Breast Surgery Department of Yulin Red Cross Hospital from May 2020 to May 2022 were selected. According to random number table method, they were divided into observation group(n=60) and control group(n=60). All were examined and diagnosed with ultrasonic diagnostic apparatus. The control group was treated with traditional open surgery, while observation group was treated with ultrasound-guided vacuum-assisted excision. The surgical related indexes, focal excision rate, serum pain stress markers [neuropeptide Y(NPY), 5-hydroxytryptamine(5-HT), prostaglandin(PGE2)], sensory nerve function in areola region, satisfaction with postoperative wound healing and postoperative complications were compared between the two groups.Results The incision length, operation time and postoperative recovery time of the observation group were shorter than those of the control group, and the blood loss was less than that of the control group(t=38.366,10.435,7.207,20.539,all P<0.001). There was no significant difference in success rate of focal resection between observation group and control group(96.67% vs. 100%, χ2=2.034,P>0.05). The levels of NPY, 5-HT and PGE2 in observation group were lower than those in control group 24 h after operation(t=4.325,3.787,4.619,all P<0.001). At 1 week and 1 month after surgery, there was no significant difference in number of patients with normal sensory nerve function, functional impairment and loss between the two groups(P>0.05). One month after operation, there was no significant difference in total satisfaction between the two groups(P>0.05). The incidence of postoperative complications(pain of affected site, skin pruritus, incision pigmentation, lesion residual) in observation group was lower than that in control group(χ2=4.615,P=0.032).Conclusion Ultrasound-guided vacuum-assisted excision can better shorten postoperative recovery time, reduce serum pain stress index, and reduce postoperative complications in patients with mammary fibroma.
  • Diagnostic value of β-hCG, ALP, and CA125 Combined with abdominal color doppler Ultraso-nography in ectopic pregnancy patients

    Objective To assess the serum expression levels of beta-human chorionic gonadotropin(beta-hCG), alkaline phosphatase(ALP), and carcinoembryonic antigen 125(CA125) in patients with ectopic pregnancy(EP), and to investigate the diagnostic value of transabdominal color Doppler ultrasound combined with beta-hCG, ALP, and CA125 for EP. Methods Seventy-nine EP patients who were admitted to the Gynecology Department of the Tibet Autonomous Region People's Hospital from June 2023 to April 2024 were selected as the case group, and 82 early pregnant women were selected as the control group. The levels of β-hCG, ALP, and CA125 in the serum of the two groups were measured, and the thickness of the endometrium and the amount of pelvic fluid were examined by transabdominal color Doppler ultrasound. The correlation between the serum β-hCG, ALP, and CA125 levels and the ultrasound parameters was analyzed by Pearson correlation analysis. The influencing factors of EP occurrence were analyzed by multivariate logistic regression analysis. The diagnostic value of transabdominal color Doppler ultrasound combined with serum β-hCG, ALP, and CA125 for EP was analyzed by receiver operating characteristic(ROC) curve. Results In the case group, the level of β-hCG, ALP were significantly lower than those in the control group(t=21.390, 19.690,P<0.001). Conversely, the levels of CA125 was notably higher(t=33.390, P<0.001). Furthermore, compared with the control group, the endometrial thickness was observed to be significantly thinner in the case group(t=14.440, P<0.001). Additionally, a positive correlation was found between endometrial thickness and serum β-hCG and ALP levels(r=0.685, 0.632; P<0.001), while a negative correlation was observed with CA125 level(r=-0.701; P<0.001). Finally, multivariate logistic regression analysis showed that endometrial thickening, high serum β-hCG levels, and high ALP levels were protective factors for ectopic pregnancy [OR(95%CI)= 0.937(0.896-0.972), 0.873(0.804-0.948), 0.077(0.034-0.097)], while high serum CA125 levels and increased pelvic fluid were risk factors for ectopic pregnancy [OR(95%CI)=1.232(1.161-1.307), 2.955(1.887-4.626)].The thickness of the endometrial lining, pelvic effusion, β-hCG, ALP, and CA125 demonstrated AUC values of 0.843, 0.802, 0.942, 0.843, and 0.916 respectively. The combined AUC of these parameters was calculated to be 0.988 which indicated a superior predictive efficacy compared to individual parameters(Z=2.370, 2.147, 2.001, 1.990, 2.676, P=0.018, 0.032, 0.039, 0.046, 0.007). Conclusion EP patients have thin endometrial lining, increased pelvic fluid, low serum beta-hCG levels, low ALP levels, and high CA125 levels. Combining abdominal color ultrasound with serum beta-hCG, ALP, and CA125 levels can improve the diagnostic efficacy of EP.
  • Effects of glutamine combined with sildenafil on vascular endothelial function and clinical efficacy in ED patients

    Objective To explore the effects of glutathione combined with sildenafil on vascular endothelial function, inflammatory response level, erectile function and clinical efficacy in patients with erectile dysfunction(ED). Methods Eighty-nine ED patients diagnosed and treated in the Urology Center of the People's Hospital of Xinjiang Uygur Autonomous Region from October 2022 to October 2023 were selected as the study objects, and were randomly divided into monotherapy group and combined treatment group, with 44 cases in the monotherapy group and 45 cases in the combined treatment group. The monotherapy group was given oral sildenafil treatment, and the combined treatment group was given oral treatment with reduced glutathione tablets. All patients were treated continuously for 1 month. The clinical efficacy, vascular endothelial function(NO, ET, VEGF, ES), inflammatory response level(hs-CRP, IL-6, IL-8, IL-10), erectile function(IIEF-5, QEQ, EHS, PSV) and incidence of adverse events were compared between the two groups before treatment, at the end of treatment and one month after treatment. Results One month after the end of treatment, the total effective rate of combined treatment group was 91.11%, which was higher than that of monotherapy group(75.00%,χ2/P=4.121/0.042). At the end of treatment and 1 month after treatment, the mean serum levels of NO and VEGF in combined treatment group were significantly higher than those before treatment and monotherapy group, and the mean ET levels were significantly lower than those before treatment and monotherapy group(t/P=5.323/<0.001, 3.683/<0.001, 3.808/<0.001, 2.615/0.011, 3.089/0.003, 3.197/0.002). Average serum hs-CRP and IL-6 levels were significantly lower than those before treatment and in monotherapy group(t/P=8.323/<0.001, 2.364/0.020, 6.787/<0.001, 2.662/0.009). Average IIEF-5, QEQ, EHS and PSV were significantly higher than before treatment, and EHS and PSV were also significantly higher than monotherapy group(t/P=6.410/<0.001, 4.066/<0.001, 8.928/<0.001, 4.532/<0.001). There was no significant difference in cure rate and incidence of adverse events between the two groups(P>0.05). Conclusion Glutathione combined with sildenafil can effectively improve vascular endothelial function and erectile function in ED patients, and significantly reduce the level of inflammation. The combine treatment scheme has remarkable clinical efficacy and safety for ED patients, and has wide clinical application prospect.
  • The mechanism of curcumin improving long-term neurobehavioral deficits in neonatal rats with hypoxic-ischemic brain injury

    Objective To investigate the pathogenesis of curcumin in reducing the neuroinflammatory and evaluate the effect of curcumin on the long-term neurobehavior in neonatal rats with hypoxic-ischemic brain damage(HIBD). Methods The experiment was conducted in the Central Laboratory of Renmin Hospital of Wuhan University from January to June 2023. A total of 59 seven-day-old Sprague-Dawley(SD) rats were selected and randomly divided into sham surgery group(n=15), hypoxic-ischemic brain damage group(HIBD, n=22), and hypoxic-ischemic brain damage + curcumin group(HIBD + curcumin, n=22) using a random number table method. The Longa score was to assess whether or not the model was successful established. Short-term neurobehavioral tests were assessed by negative geotaxis reflex and righting reflex at 48 h after modeling. The long-term neurobehavior were assessed by the cylinder test, morris water maze, corner turn test, and pole-climbing test at 4 weeks after modeling. The brain tissues were collected to detect the levels of inflammatory factors(TNF-α, IL-1β, and IL-6) were detected by Elisa kit and the protein levels(STAT3 and p-STAT3) were detected by Western blot. Results The Longa scores of 1-3 in neonatal rats in the HIBD group(n=18) and the HIBD + curcumin group(n=15) suggested successful modeling. The short-term neurobehavior tests showed that compared to the HIBD group, the HIBD + curcumin group had a shorter negative geotaxis reflex and righting reflex time(F/P=135.037/<0.001, 86.711/<0.001). The long-term neurobehavioral tests revealed significant improvements in the HIBD + curcumin group compared to the HIBD group. The HIBD + curcumin group exhibited increased utilization of the left forelimb in the cylinder test, shortened latency stage to cross the platform in the water maze test, increased number of crossings in the platform quadrant, decreased scores in the corner test, and reduced climbing time in the pole test(F/P=55.671/<0.001, 102.160/<0.001, 61.455/<0.001, 28.526/<0.001, 51.643/<0.001). ELISA detection showed that compared with the HIBD group, the HIBD + curcumin group exhibited a significant decrease in the levels of TNF-α, IL-1β, and IL-6 in the brain(F/P=424.753/<0.001, 637.606/<0.001, 361.432/<0.001). Protein immunoblotting technology showed that compared with the HIBD group, the HIBD + curcumin group significantly decreased the expression levels of p-STAT3/STAT3 in the brain(F/P=26.473/<0.001). Conclusion Curcumin can relieve neuroinflammation and improve the long-term neurobehavioral dysfunction by inhibiting STAT3 phosphorylation in HIBD neonatal rat.
  • The protective effect of Shensong Yangxin Capsule on human microvascular endothelial cells damaged by high glucose and high fat

    Objective To investigate the intervention effect of microvascular endothelial cell injury induced by high glucose and high fat and the intervention effect of Shensong Yangxin Capsule(SSYX). Methods The experiments were conducted from June 2022 to February 2023 at the State Key Laboratory for Innovation and Transformation of Luobing Theory. Human microvascular endothelial cells were randomly divided into the normal group, the model group, the SSYX-L, the SSYX-M and the SSYX-H group(0.25,0.5,1.0 mg/L). In addition to the normal group, the other 4 groups were treated with high glucose and high fat to establish the human microvascular endothelial cell injury model, and carried out corresponding intervention.ELISA was used to detect cell activity, fluorescence microscopy was used to detect cellular mitochondrial membrane potential, and Western blot was used to detect protein levels of DRP1, MFN2, GRP78, NF-κB p65, P-selectin, E-selectin, and IL-6. Results Compared with the normal group, the cell survival activity and mitochondrial membrane potential of the model group were significantly reduced, the protein expressions of DRP1, GRP78, NF-κB p65, P-selectin, E-selectin and IL-6 were significantly increased, and the expression of MFN2 protein was significantly decreased. Compared with the model group, the cell survival activity and mitochondrial membrane potential of each dose group were significantly increased(P<0.01), and the protein expressions of GRP78, NF-κB p65 and E-selectin were significantly decreased(P<0.01 or <0.05). The expression of MFN2 protein in the SSYX-H group was significantly increased(P<0.05), and the expression of IL-6 protein was significantly decreased(P<0.05), and the expression of DRP1 and P-selectin protein in the SSYX-M and SSYX-H groups was significantly decreased(P<0.05 or <0.01). Conclusion Shensong Yangxin Capsule can improve the survival activity of microvascular endothelial cells, maintain mitochondrial homeostasis, reduce endoplasmic reticulum stress, inhibit inflammatory response, and reduce the level of adhesion molecules, thereby protecting microvascular endothelial cells induced by high glucose and high fat.
