  • Study on the correlation between serum FGF23 and D-ser levels and hearing impairment in maintenance hemodialysis patients

    ObjectiveTo investigate the correlation between serum fibroblast growth factor 23(FGF23) and D-serine(D-ser) levels and hearing impairment in maintenance hemodialysis(MHD) patients.MethodsForty MHD patients with combined hearing impairment admitted to the Department of Nephrology of Shanghai Fifth People's Hospital Affiliated to Fudan University from January 2021 to December 2023 were selected as the hearing-impaired group, 40 MHD patients without combined hearing impairment were selected as the non-hearing-impaired group, and 40 healthy patients with physical examination were selected as the healthy control group; Serum FGF23 and D-ser levels were measured by enzyme-linked immunosorbent assay with high performance liquid chromatography fluorescence; The correlation between serum FGF23 and D-ser levels and PTA in MHD patients was analyzed by Spearman/Pearson correlation coefficients, the factors of hearing impairment in MHD patients were analyzed by multifactorial logistic regression, and the predictive value of serum FGF23 and D-ser levels for hearing impairment in MHD patients was analyzed by ROC curves.ResultsSerum FGF23 and D-ser levels were higher in the hearing-impaired group than in the non-hearing-impaired group and the control group, and serum FGF23 and D-ser levels were higher in the non-hearing-impaired group than in the control group(F/P=69.767/<0.001, 307.552/0.001). Spearman/Pearson correlation showed a positive correlation between serum FGF23 and D-ser levels and PTA in patients with MHD(rs/P=0.649/<0.001,0.669/<0.001). Multifactorial logistic regression showed that prolonged dialysis age, elevated FGF23, and elevated D-ser were independent risk factors for hearing impairment in MHD patients [OR(95%CI)=1.036(1.007-1.065), 1.018(1.006-1.029), and 1.526(1.145-2.033)]; The ROC curves showed that the area under the curve(AUC) of serum FGF23, D-ser levels and the combination of the two predicted hearing impairment in MHD patients were 0.773, 0.788, 0.880, respectively, and that the combination of the two was greater than the AUCs predicted by each of the serum FGF23 and D-ser levels alone(Z=2.409, 2.344, P=0.016, 0.019).ConclusionElevated serum FGF23 and D-ser levels in MHD patients are independent risk factors for hearing impairment, and the combined serum FGF23 and D-ser levels are of high value in predicting hearing impairment in MHD patients.
  • Expression and prognostic value of serum LncRNA PVT1, miR-486-5p in patients with acute myeloid leukocyte

    ObjectiveTo explore the relationship between the serum expression levels of long non-coding RNAPVT1 gene(LncRNAPVT1) and microRNA-486-5p(miR-486-5p) in plasma and the pathological characteristics and prognosis of acute myeloid leukemia(AML) patients.MethodsOne hundred patients with AML were selected as AML group and 100 healthy subjects were selected as healthy control group. The relative expression levels of serum LncRNA PVT1 and miR-486-5p were detected by real-time fluorescence quantitative polymerase chain reaction(RT-qPCR). Pearson correlation method was used to evaluate the correlation between serum LncRNA PVT1 and miR-486-5p in AML patients and its relationship with pathological characteristics. Kaplan-Meier method was used to analyze the survival curve of AML patients, and Cox regression analysis was used to evaluate the risk factors affecting the prognosis of AML patients.ResultsThe levels of LncRNA PVT1 in AML experimental group patients were higher than those in the healthy control group. the levels of miR-486-5p in AML experimental group patients were lower than those in the healthy control group(t/P=15.403/<0.001、18.305/<0.001). Pearson correlation analysis showed that LncRNA PVT1 was negatively correlated with miR-486-5p level in AML group(r=-0.261,P<0.01). There were statistically significant differences in leukocyte count, bone marrow original cell ratio, prognostic risk stratification, LncRNA PVT1 and miR-486-5p levels in AML group(LncRNA PVT1:t/P=2.934/0.004,2.901/0.005,10.985/<0.001;miR-486-5p: t/P=2.598/0.012,2.604/0.012,8.593/<0.001). The 3-year overall survival rate of those with high expression of LncRNA PVT1 was lower than those with low expression of LncRNA PVT1(χ2/P=14.160/<0.001), and the 3-year overall survival rate of those with high expression of miR-486-5p was higher than that of those with low expression of miR-486-5p(χ2/P=16.820/<0.001). Comprehensive analysis of study data showed that high level of LncRNA PVT1 expression and low level of miR-486-5p expression were risk factors for poor prognosis in patients with acute myeloid leukemia[RR(95%CI)=1.982(1.148-2.993),1.668(1.085-2.641),1.659(1.068-2.602)].ConclusionThe high expression of LncRNA PVT1 and low expression of miR-486-5p in AML patients are related to white blood cell count, bone marrow primitive cell ratio, risk stratification of prognosis and patient prognosis.
  • Effects of Tongxinluo Capsule on sepsis and microcirculation parameters

    ObjectiveTo observe the effects of Tongxinluo Capsule on major circulation parameters(MAP, CVP and ScvO2), micro circulation parameters(Pcv-aCO2, Pcv-aCO2/Ca-cvO2, Lac, CRT), clinical efficacy, APACHE-Ⅱ, SOFA score, mechanical ventilation time, ICU admission time and 28-day mortality in septic patients.MethodsSixty patients with sepsis were randomly divided into treatment group and control group, 30 cases in each group, both groups were given conventional western medicine, the treatment group plus Tongxinluo Capsule, the control group plus placebo, 4 capsules, 3 times a day, the course of treatment for 7 days. Blood samples were collected at 0, 6, 24, 48 and 72 hours after ICU admission respectively, and relevant research indexes were detected.ResultsThe total effective rate of treatment group was significantly higher than that of control group(100.0% vs. 66.7%,χ2=8.857, P=0.003). The intra-group comparisons of large cycle parameters MAP, CVP and ScvO2at different time points of 0h, 6h, 24h, 48h and 72 h in the both groups were all statistically significant(all P<0.001), and="" no="" significance="" between="" the="" two="" all="" p="">0.05). In the control group, Pcv-aCO2was significantly different in the above 5 different time points(P=0.006), but no significance in Pcv-aCO2/Ca-cvO2(P=0.261). The intra-group comparisons of Pcv-aCO2and Pcv-aCO2/Ca-cvO2in treatment group were all statistically significant(all P<0.05). There were no significances in Pcv-aCO2and Pcv-aCO2/Ca-cvO2between the 2 groups at 0h, 6h and 24h(all P>0.05), but there were significances between the 48h and 72h(all P<0.05), and the treatment group was superior to the control group. Lac and CRT were statistically significant in the above 5 different time points with intra-group comparisons of the both groups(all P<0.001). 2="" 48="" there="" were="" no="" significances="" in="" lac="" levels="" between="" the="" groups="" at="" 24h="" and="" all="" p="">0.05), but there was a statistically significant difference between the groups at 72 h(P=0.036), and the treatment group was better than the control group. No significances were found in CRT between groups at 0h, 6h and 24h(all P>0.05), but there were significances between groups at 48h and 72 h(all P<0.05), and="" the="" treatment="" group="" was="" superior="" to="" control="" group.="" there="" were="" no="" significant="" differences="" in="" sofa="" scores="" between="" two="" groups="" before="" p="" all="">0.05). There were significant differences before and after treatment with intra-group comparison of both groups(all P<0.001). 7="" after="" days="" of="" there="" was="" no="" significant="" difference="" in="" scores="" among="" p="">0.05), while significant difference was found in SOFA scores among groups(P=0.010). The treatment group was better than the control group. But there were no significant differences in mechanical ventilation time, ICU admission time and 28-day mortality between the two groups(all P>0.05).ConclusionTongxinluo Capsule can improve the microcirculation disorder of sepsis patients, relieve the condition and improve the clinical efficacy of TCM syndromes, but it cannot shorten the mechanical ventilation time, ICU admission time and 28-day mortality.
  • Clinical significance maternal serum GDF-15 and ANGPTL8 for prenatal diagnosis of fetal congenital heart disease

    ObjectiveTo investigate the expression of serum growth differentiation factor-15(GDF-15) and angiopoietin-like protein 8(ANGPTL8) in the mother with fetal congenital heart disease(CHD), and the value of them combined in the diagnosis of fetal CHD.MethodsA total of 120 pregnant women suspected of fetal CHD who underwent prenatal screening in our hospital from January 2020 to March 2023 were collected as research subjects. They were separated into CHD group(n=86) and non-CHD group(n=34) based on the results of Color Doppler echocardiography. The expression levels of serum GDF-15 and ANGPTL8 were compared between the two groups; Multivariate Logistic regression was used to analyze the influencing factors of fetal CHD; ROC curve was applied to analyze the diagnostic efficacy of serum GDF-15 and ANGPTL8 in fetal CHD.ResultsCompared with the non CHD group, the serum levels of GDF-15 and ANGPTL8 in the CHD group were obviously increased(t/P=7.860/<0.001, 7.334/<0.001). Multivariate Logistic regression analysis showed that gravidity, family history of CHD, GDF-15 and ANGPTL8 were the influencing factors of fetal CHD[OR(95%CI)=1.383(1.082-1.767),1.596(1.148-2.218),3.596(1.694-7.633),3.175(1.571-6.417)]. The AUC of serum GDF-15,ANGPTL8 and their combination in the diagnosis of fetal CHD was 0.805,0.835 and 0.903, the combination of serum GDF-15 and ANGPTL8 in the diagnosis of fetal CHD was better than GDF-15. ANGPTL8 was independently predicted(Z=2.052, 2.219,P=0.040, 0.027).ConclusionThe maternal serum GDF-15 and ANGPTL8 levels in fetal CHD are obviously elevated. Color Doppler echocardiography combined with maternal serum GDF-15 and ANGPTL8 has high diagnostic potential for fetal CHD.
  • Diagnostic value of combination of serum AIF-1, Bmi-1, and MAP19 in patients with cervical cancer

    ObjectiveTo investigate the expression levels of allograft inflammatory factor-1(AIF-1), B-cell specific moloney leukemia virus insertion site 1(Bmi-1), and mannose-binding lectin associated protein 19(MAP19) in serum, and their combined diagnostic value in cervical cancer patients.MethodsA total of 180 cervical cancer patients admitted from January 2023 to December 2023 were regarded as the observation group, and another 180 healthy volunteers who underwent physical examinations during the same period were selected as the control group. Enzyme linked immunosorbent assay(ELISA) was applied to detect the expression levels of AIF-1, Bmi-1, and MAP19 in the serum of two groups. The expression levels of AIF-1, Bmi-1 and MAP19 in different stages of cervical cancer were compared. Spearman was applied to analyze the relationship between the expression levels of AIF-1, Bmi-1, MAP19 in serum with the incidence of cervical cancer. Receiver operating characteristic(ROC) curve was plotted to analyze the diagnostic value of serum AIF-1, Bmi-1, and MAP19 expression levels for cervical cancer.ResultsCompared with the control group, the expression levels of serum AIF-1, Bmi-1, and MAP19 in the observation group were increased(t=13.054, 13.598, 10.601,P<0.001); The expression levels of AIF-1, Bmi-1 and MAP19 in serum of patients with cervical cancer increased with the increase of stage(F=32.001, 8.232, 10.602,P<0.001). the proportion of high expression levels of serum AIF-1, Bmi-1, and MAP19 in cervical cancer patients increased in pathological low differentiation, HPV positivity, FIGO stages III-IV, and lymph node metastasis, while the proportion of low expression levels increased in pathological moderate to high differentiation, HPV negativity, FIGO stages Ⅰ-Ⅱ, and no lymph node metastasis(AIF-1:χ2=41.162, 27.607, 13.718, 23.824,P<0.001, Bmi-1:χ2=33.563, 22.060, 22.599, 18.451,P<0.001, MAP19:χ2=49.585, 14.913, 25.545, 13.605,P<0.001); The AUC for predicting cervical cancer lesions by serum AIF-1, Bmi-1, MAP19 expression levels, and their combination were 0.759, 0.726, 0.751, and 0.839, respectively, with the combination being better than each individual prediction(Z=2.499, 3.363, 2.749,P=0.012, <0.001, 0.016).ConclusionThe expression levels of AIF-1, Bmi-1, and MAP19 in the serum of cervical cancer patients are obviously increased, the expression levels of AIF-1, Bmi-1 and MAP19 in serum increased with the increase of cervical cancer stage, and the combination of the three has high diagnostic efficacy for cervical cancer.
